AIRCRAFT ACCIDENT REPORT

ARCTIC-PACIFIC, INC., CURTISS WRIGHT, SUPER C-46F, N 1244N, TOLEDO EXPRESS AIRPORT, TOLEDO, OHIO OCTOBER 29, 1960

MENDED REPORT

The above-entitled accident report, adopted January 15, 1962, is hereby amended as follows:

1.

Insert the following language in lieu of the second paragraph on page 12 of the double spaced report (the last paragraph on page 7 of the single spaced report):

In the investigation of this accident it was disclosed that FAA policy regarding inspections of supplemental air carriers, pursuant to Section 42.8 of Part 42 of the Civil Air Regulations provided, among other things, quarterly en route operations inspections and an annual en route maintenance inspection. However, neither the Operations nor Maintenance FAA Air Carrier Inspectors assigned to Arctic-Pacific conducted any of these inspections during the preceding eleven months of operations because of duties with other assigned carriers. During this eleven-month period, the FAA did conduct nine ramp maintenance inspections and seven ramp operations inspections under an FAA policy of conducting at least one ramp inspection each month. In addition, the FAA conducted three main base maintenance inspections, one main base operations inspection, and the full, annual pre-certification inspection of October 11-13, 1960.

Delete the last paragraph on page 16 of the double spaced report (the third complete paragraph on page 10 of the single spaced report).
